I am aware that participation in a fitness activity, sports, yoga (the “Activity”), energetic modalities, in private or group gathering may result in accident or injury, and I assume the risk connected with the participation in this Activity. I represent that I am in good health, cleared by a doctor to participate in physical activities, and that I do NOT suffer from a physical impairment which would limit my ability to participate in any activity sponsored or offered by: Ventura Pop Up Yoga, Venture Well Yoga, LLC, or their affiliates (“Ventura Pop Up Yoga”). I acknowledge that Ventura Pop Up Yoga and/or their agents, affiliates, or partners, have not and will not, render any medical services, including but not limited to a medical diagnosis of a physical condition. I specifically agree that Ventura Pop Up Yoga or their agents, affiliates, partners, or participants, shall not be liable for any claim, demand, injury, illness, or cause of action of any kind resulting from or related to my use of facilities or equipment or participation in any gathering, fitness activity, sport, exercise or yoga within or around this Activity or event, and that I agree to hold Ventura Pop Up Yoga, Jessy Raspiller, and its affiliates and partners harmless from same.

I agree that any photos or videos taken by Ventura Pop Up Yoga or its agents are the property of Ventura Pop Up Yoga and may be used for marketing purposes, posts and publications as seen appropriate by Ventura Pop Up Yoga and its agents.

I am over the age of 18, have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
